The aging-related anthropometric changes, including brow ptosis, periocular hooding, soft tissue descent, and dermal atrophy are known to have an effect on the results of PMU, but there are very few original clinical studies describing how technique may be altered for an aging patient. The current retrospective observational study examined 142 female clients (mean age 61.3 ± 9.2) who received anthropometrically guided PMU treatments (2021-2025). Prior to the procedure, females’ eyebrows, periocular region, and skin quality were assessed through systematic measurements. Results were reviewed at various follow-up intervals. Amongst aging the areas with the most statistically significant (p<0.001) increase in severity were brow ptosis and skin atrophy. Powder and nano techniques resulted in the most pigment retention (85.2%) followed by microblading (64.4%) atrophic skin. Anthropometric-adapted mapping had the highest population-wide symmetry (88.7%), submitted client satisfaction scores (mean 4.6/5) along with the lowest complication rate (7.0%). Aged facial anthropometry when incorporated systematically into PMU procedures may lead to optimum safety, retention, and aesthetic results in an aging client.
